Key Responsibilities\n

• Collaborate with the Social Work Manager to conduct thorough evaluations of patients, understanding their psychosocial needs and challenges related to mental and physical illnesses.

• Utilize the NASW Code of Ethics to guide all clinical interactions, ensuring that professional standards of practice are adhered to at all times.

• Complete detailed bio-psychosocial assessments to identify barriers to treatment and discharge; develop actionable plans to enhance patient health outcomes.

• Provide training and consultation to medical and support staff about patients' psychosocial backgrounds to assist in formulating patient care programs.
\n
• Develop and implement individualized treatment plans using both individual and group therapy modalities while monitoring and analyzing treatment effectiveness.

• Document all social work activities comprehensively according to departmental standards, ensuring accurate medical recordkeeping for each patient.
\n
• Educate patients and their families about medical and social work interventions, fostering understanding and encouraging adherence to recommended care plans.

• Assess patients’ advocacy needs while facilitating community-based support initiatives, emphasizing the empowerment and self-determination of clients.

• Collect case-related statistics and prepare reports to comply with departmental expectations and inform continual service improvement.

• Maintain and enhance your professional expertise through ongoing education and remain informed about community resources relevant to the patient population.

• Comply with all relevant laws, regulations, and organizational policies to help maintain a safe and ethical treatment environment.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

• A valid and current license as a Licensed Clinical Social Worker (LCSW) issued by the State Board of Social Work Licensure.

• At least one year of clinical experience in a health or mental health setting, with a focus on working with diverse populations.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 85,000 to 100,000 annually, along with a sign-on bonus for qualified candidates.
