Key Responsibilities\n

• Act as a liaison between families and outpatient providers to ensure comprehensive care for clients.

• Engage with insurance companies to manage pre-certification processes and utilization management effectively.

• Participate proactively in discharge planning and aftercare strategies to ensure seamless transitions for clients.

• Provide therapeutic meal support, advocating for and modeling healthy relationships with food, especially for clients facing eating disorders.

• Deliver safe, empathetic, and affirming care tailored to the unique circumstances of each client, considering their intersecting identities and backgrounds.

Patient Population / Client Focus\n

You will work primarily with individuals navigating various mental health challenges, including but not limited to mood disorders, anxiety disorders, and psychosocial issues related to aging. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is essential for this role.
