Work Environment

This is a remote position, offering flexibility and convenience for your work-life balance while ensuring you remain connected with our team and clients. Our normal operating hours are Monday through Friday, from 8:00 AM to 4:30 PM, with some occasional evening or weekend hours to accommodate the needs of our clients. On-call availability may be required, ensuring we are responsive to urgent situations that arise outside regular business hours. Your role will involve conducting virtual sessions, participating in remote collaboration with interdisciplinary teams, and engaging with clients through various digital platforms.

Key Responsibilities

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting comprehensive psychosocial assessments to identify emotional, mental, and social needs of senior clients.
• Collaborating closely with clients and their families to create personalized care plans that reflect individual goals and preferences, adapting to any changes in their needs.
• Providing counseling and emotional support, aiding clients in navigating significant life transitions and challenges such as bereavement or health crises.
• Serving as an advocate for clients, ensuring their rights are upheld while helping them access essential community resources and services.
• Working collaboratively with interdisciplinary teams—including healthcare professionals, nurses, and caregivers—to enhance care strategies and outcomes for clients.
• Monitoring clients' well-being through regular check-in sessions, addressing living conditions, and responding to any safety concerns.
• Facilitating healthy communication among family members involved in care, helping to resolve conflicts when they arise.
• Educating clients and families about available community support programs and resources that foster social engagement and quality of life improvements.
• Maintaining meticulous documentation of client interactions, evaluations, and care plans, adhering to best practices in record-keeping and compliance standards.
• Staying informed about current social work practices, policies, and regulations, ensuring ethical compliance in all interactions.

Patient Population / Client Focus

Your primary focus will be on supporting senior clients who face unique challenges related to aging, mental health issues, and family dynamics. Many clients experience anxiety, depression, or transitions brought on by changing health statuses, and your role is integral in guiding them toward stability and improved emotional resilience. You will also work with their families to equip them with understanding and resources to better support their loved ones.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in social work, preferably within a community setting that serves seniors or geriatric populations.
• Strong foundation in social work principles and aging-related issues, along with an understanding of available community resources.
• Exceptional assessment, counseling, and problem-solving abilities.
• Demonstrated empathy, compassion, and the ability to establish strong rapport with clients and their families.
• Excellent communication and interpersonal skills, facilitating effective collaboration with diverse healthcare teams.
• Proficiency in electronic health record (EHR) systems and familiarity with modern computer applications.

Preferred Experience

While not mandatory, experience in crisis intervention, and familiarity with therapeutic techniques and best practices are strongly preferred. Additionally, previous work in multidisciplinary teams will be beneficial in this position.

Compensation and Benefits

The position offers a competitive salary ranging from $90,000 to $100,000 annually. In addition to the base salary, you will receive a comprehensive benefits package, including:
