Key Responsibilities\n

• Utilize telehealth platforms to perform individual counseling sessions, facilitating a safe space for clients to share and work through their challenges.

• Coordinate with county crisis services by maintaining accurate crisis alerts to ensure seamless support for participants facing urgent challenges.

• Document transitions for clients moving to hospital or inpatient services, ensuring effective care coordination with hospital staff to facilitate safe discharges.

• Participate in regular team meetings to discuss case management strategies, client progress, and to receive ongoing supervision and support.

• Perform additional duties as assigned, tailored to the needs of the organization and its participants.

Patient Population / Client Focus\n

In this role, you will primarily work with a varied clientele that may include individuals facing mental health challenges such as anxiety, depression, trauma, and substance use disorders. Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

Compensation and Benefits\n
