Key Responsibilities\n

• Proactively engage with participants to prevent crises and minimize the need for emergency services, effectively utilizing telehealth technology.

• Maintain detailed crisis alerts and crisis management plans, ensuring seamless coordination with county crisis services for timely interventions.

• Manage transitions for clients needing inpatient services to outpatient support, coordinating closely with hospital staff to facilitate smooth discharges and transfers.

• Document all interactions and treatment plans accurately in our electronic health record (EHR) system to ensure comprehensive patient care continuity.

• Monitor patients' progress towards their goals, providing evidence-based counseling methods tailored to individual needs.

Patient Population / Client Focus\n

In this role, you will work with a diverse population of clients experiencing an array of mental health challenges, from anxiety and depression to more complex behavioral issues. Our participants come from various cultural and socioeconomic backgrounds, providing a rich tapestry of experiences that require sensitivity and adaptability. Qualifications\n

• Masters degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling.

Compensation and Benefits\n

We offer a competitive salary range of $100,000 to $105,000 per year, which is commensurate with your experience and caseload.
