Key Responsibilities\n

• Guarantee the effective implementation of individualized program plans, modifying approaches as required to meet evolving client needs.

• Actively engage in Interdisciplinary Team Meetings, sharing insights and updates regarding client management strategies.

• Complete regular progress notes, both monthly and quarterly, to document developments and inform future therapeutic actions.

• Facilitate training sessions designed to empower staff in the delivery of program plans and documentation processes.

• Leverage follow-up interviews and home visits to reinforce therapeutic support for clients and ensure accessibility of resources.

• Maintain meticulous records that keep all relevant information accessible to team members and external parties as needed.

• Serve as a liaison between the organization and the mental health community to enhance support networks and developmental goals.
