Description

American Traveler is seeking a Physical Therapist for a 13-week home health assignment in a rural, multi-county service area, requiring at least 1 year of experience and active BLS certification.

Details

• Home health care setting serving four counties (Noble, Lagrange, Steuben, and DeKalb), primarily rural with mid-sized towns
• Caseload equivalent to approximately 46 patient visits per day using a point-based tracking system
• Common cases include elderly falls and fractures
• Responsibilities include Oasis Assessments and Start of Care cases
• Charting system: Homecare Homebase (HCHB)
• Tablet used for time and visit tracking
• Schedule: MondayFriday, 8:00 AM5:00 PM; day shift, 8-hour shifts

Requirements

• Minimum 1 year of physical therapy experience required
• Active PT state license or Compact (NLC) licensure required; pending licenses will not be accepted license required if applicable
• Active BLS (AHA) certification required; pending certifications will not be accepted
• Valid driver's license required for consideration
• 2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire

Additional Information

• 12-hour orientation provided
• Attire: business casual or hunter green scrubs with a white or black top
• 13-week assignment with a standard 13-week cancellation policy
