Key Responsibilities\n

• Conduct comprehensive assessments for patients to identify their mental health needs, ensuring tailored treatment plans.

• Provide a detailed schedule of appointments, including home visits and virtual sessions, to facilitate client engagement and track advancements.

• Coordinate and conduct home visits when required, integrating in-home observations and data analysis to enrich client understanding and care.

• Oversee the implementation of treatment plans, guaranteeing that all interventions are executed effectively and are beneficial to the client's progress.

• Participate actively in Interdisciplinary Team Meetings to discuss treatment outcomes and develop collective strategies.

• Document monthly and quarterly notes summarizing patient progress and identifying future needs or changes in the course of treatment.

• Provide training and certification for team members to ensure competency in program implementation and documentation practices.

• Facilitate follow-up sessions, which may include interviews, home visits, or continued observations to ensure success and support for your clients.

• Maintain accurate records that are readily accessible to authorized individuals, ensuring confidentiality and compliance with all legal standards.
