Work Environment\n

This position is entirely remote, allowing for flexibility while maintaining a structured work schedule from Monday to Friday, 9:00 AM to 5:00 PM. As part of a dedicated team of social workers, counselors, and health professionals, you will collaborate closely with peers and management leveraging technology and digital tools to communicate effectively and fulfill client needs efficiently. Utilizing electronic health records (EHR) systems, you will document assessments and interventions, ensuring compliance with all regulatory standards.

Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments for clients to determine their mental health, substance abuse, and medical needs within a primary care context.

• Document mental health evaluations, progress notes, crisis interventions, and treatment plans in an electronic health record system to ensure accurate and secure patient data management.

• Deliver individual and group counseling services to diverse client populations, addressing their unique psychological and social needs.

• Provide referrals to local community resources and support programs, such as medication assistance, that can facilitate the client’s recovery and improve their access to care.

• Work collaboratively with quality improvement teams to develop and implement effective data collection processes, contributing to ongoing assessment and enhancement of service delivery.

• Participate in interdisciplinary team meetings to discuss patient progress, share insights, and strategize on care plans tailored to individual client situations.

Qualifications\n

Compensation and Benefits\n
