Key Responsibilities\n

As a Telehealth Licensed Professional Counselor at Big Leap Family Guidance Centers, you will play a critical role in enhancing our patients' mental health and well-being through a variety of responsibilities, including:

• Conducting thorough patient intakes to diagnose and evaluate individuals' mental health needs, and offering tailored treatment recommendations to primary care providers.

• Providing brief, focused therapeutic interventions for patients needing mental health services via telehealth sessions, ensuring quality care and support.
\n
• Working collaboratively with primary care providers to identify patients at risk for mental health challenges, ensuring timely referrals to specialized services when necessary.
\n
• Implementing, monitoring, and updating treatment and relapse prevention plans, allowing patients to maintain stability in their mental health.
\n
• Documenting patient progress accurately in treatment charts, keeping thorough and organized records that support ongoing patient evaluations.
\n
• Engaging in regular follow-up communications with patients to assess their progress and adjust treatment strategies as needed.
\n
• Providing educational resources and self-management skills that empower patients to take charge of their mental health and recovery.

• Consulting with clinic management and interdisciplinary team members regarding behavioral health services, helping shape outcome evaluations and program improvements.

Qualifications\n

To succeed in this role, candidates should meet the following qualifications:

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Current Licensed Professional Counselor (LPC) license in Connecticut.
