Key Responsibilities\n

• Deliver high-quality mental health care through an 8-week evidence-based program remotely, focusing on client engagement and therapeutic outcomes.

• Regularly monitor client progress by analyzing data and documenting session notes while providing responsive follow-up communications and care coordination recommendations.

• Collaborate with a multidisciplinary care team to assist clients in accessing necessary resources, which includes guiding them through consent and authorization processes.

• Identify and manage high-risk encounters with clients who demonstrate elevated safety concerns, strictly adhering to organizational protocols.

• Engage in routinely scheduled meetings with supervisors, partake in monthly team discussions, and contribute to Town Hall events to support team cohesion and share insights.

• Ensure the confidentiality and privacy of client information in compliance with HIPAA regulations while fulfilling annual training requirements.

Patient Population / Client Focus\n

Qualifications\n

• Active licensure as a Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
