As an integral part of our hospice team, you will conduct your counseling and therapeutic sessions via telehealth platforms, ensuring that you can effectively reach clients in their own environments. • Conduct comprehensive assessments that enable us to identify clients' social, emotional, and psychological needs through effective telehealth platforms.

• Provide individualized counseling and support to clients and their families, addressing a wide spectrum of personal challenges including grief, loss, and managing chronic conditions.

• Deliver both individual and group counseling sessions utilizing evidence-based therapeutic approaches tailored to various client backgrounds and needs.

• Develop and implement service plans aligned with clients' goals, regularly reviewing progress and adapting as necessary to ensure optimal outcomes.

• Advocate for clients' rights and navigate them through complex resource systems, providing insights on social justice issues impacting their well-being.

• Contribute actively to program development initiatives, evaluation metrics, and necessary grant reporting, ensuring that our projects are aligned with best practices and community needs.

• Engage in collaboration with community organizations, government agencies, and volunteer groups to create an expanded support network for our clients.

• Participate in team meetings, training sessions, and community outreach activities to promote our services and expand our impact.

At Catalyst Behavioral Health Center, the primary focus of your work will be with a diverse patient population at our hospice center who may be experiencing significant emotional distress due to loss, terminal illness, or other health crises. Your compassionate approach will be crucial in supporting clients and their families, helping them navigate through one of the most challenging periods of their lives while facilitating coping strategies and resilience.

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).
\n
• Strong communication and organizational skills, coupled with a deep capacity for empathy and commitment to ethical practices in social work.

• Experience in nonprofit organizations or similar supportive environments is preferred.

• Familiarity with social justice issues, advocacy strategies, trauma-informed care practices, and community resource navigation.

We offer a competitive salary ranging from USD 78,000 to USD 115,000 annually, depending on your experience and caseload responsibilities.
