Key Responsibilities\n

• Conduct thorough assessments of clients to determine their mental health needs and develop tailored treatment plans that address respective concerns.

• Maintain constant communication with supervisors, direct care teams, families, and guardians to keep all parties informed about clients’ progress and treatment modifications.

• Schedule and organize ongoing appointments, home visits, and necessary observations to ensure effective monitoring of clients’ advancements.

• Supervise and coordinate in-home visits, ensuring compliance with established program standards and producing insightful reports based on clinical observations.

• Facilitate interdisciplinary team meetings and contribute to discussions regarding treatment strategies and best practices.

• Document client progress in monthly and quarterly reports, ensuring comprehensive and clear records that can be shared with relevant stakeholders.

• Train and support colleagues in implementing the program plans effectively, boosting team competency in clinical documentation and care strategies.

Qualifications\n
