Work Environment\n

This role is fully remote, allowing flexibility while collaborating with a team that prioritizes communication and support. Our Community Health Clinic serves the local population as well as families remote, offering a variety of services designed to enhance child development. You will engage with a multidisciplinary team of professionals committed to holistic care and evidence-based practices.

Key Responsibilities\n

As a Licensed Clinical Social Worker, you will:

• Conduct thorough evaluations and assessments of children aged 0-3 years to identify developmental needs and create actionable plans.
\n
• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), working collaboratively with families and team members.
\n
• Monitor progress and outcomes for children under your care and adjust strategies as needed based on evidence-based methodologies.
\n
• Provide educational and emotional support to families, helping them to understand their child’s developmental journey.
\n
• Utilize a coaching model within natural learning environments to foster skill acquisition in children with developmental delays or disabilities.
\n
• Conduct follow-ups and maintain compliance with Federal and State regulations, ensuring the highest standards of care.

Patient Population / Client Focus\n

In this position, you will primarily work with young children exhibiting disabilities or developmental delays, as well as their families. Your efforts will focus on promoting early intervention strategies that enhance physical, cognitive, social/emotional, and communication skills during these critical developmental years. You will champion the family’s role in this process, empowering them with knowledge and support.

Qualifications\n

We require the following qualifications for this role:

• A Master's Degree in Social Work from an accredited institution.
\n
• Current licensing as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
\n
• A valid driver's license to facilitate transportation when required.
\n
• Experience working with children in an early intervention context is essential.

• Strong organizational and time management skills, important for maintaining effective caseloads.

• Excellent communication skills, both verbal and written, to facilitate meaningful interactions with families and team members.

Compensation and Benefits\n
