Key Responsibilities\n

• Conduct thorough psychosocial assessments to ascertain the mental, emotional, and behavioral health needs of clients.

• Develop and implement individualized treatment plans based on assessment outcomes, adapting care strategies as needed based on ongoing evaluations of client progress.

• Collaborate effectively with clients' families, healthcare teams, and community resources to ensure a holistic approach to care and to facilitate continuity of services.

• Diagnose mental health disorders and deliver evidence-based therapies while providing referrals to additional resources like community programs, support groups, or specialized providers as necessary.

• Facilitate discharge planning with multidisciplinary team members to maintain an unwavering focus on patient-centered care during transitions.

• Maintain compliance with all state, federal, and clinical regulations in line with the Mental Health Practice Act.

• Track daily responsibilities by managing caseloads responsively, ensuring timely interventions and comprehensive support throughout treatment journeys.

• Perform consultations with patients and families to provide necessary social work interventions and to offer supportive guidance during times of crisis, bereavement, and other personal hardships.

• Serve as a patient advocate, ensuring clear communication across various teams and agencies to address client needs effectively.

• Identify potential barriers that could impact client progress or discharge, working with colleagues to streamline solutions.

Compensation and Benefits\n

Elevance Counseling offers a competitive salary ranging from $90,000 to $100,000 annually, reflective of experience and qualifications. In addition, we provide a sign-on bonus of up to $1,000 as an incentive for joining our dedicated team.
