As a remote position based out of Chambersburg, Pennsylvania, you will work closely with a multidisciplinary team comprised of healthcare professionals dedicated to providing comprehensive hospice care. Key Responsibilities\n

• Conduct thorough intake and psychosocial assessments, establishing a comprehensive understanding of each patient’s emotional and social needs.

• Collaborate with the treatment team to develop, present, and revise personalized treatment plans that address the goals and challenges of patients residing in hospice care.

• Engage in clinical case conferences to discuss treatment progress, barriers, and necessary adjustments, ensuring seamless continuity of care.

Qualifications\n

• A Master’s Degree in Social Work (MSW) or a related field is mandatory.

• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.

Compensation and Benefits\n
