Position Overview

The Integrative Health and Wellness Clinic is seeking a dedicated and skilled Nurse Practitioner (NP) to join our dynamic outpatient practice within the Center for Advanced Cardiac Care (CACC) located in Buffalo, New York. This full-time position will empower you to work collaboratively within a multidisciplinary team led by the Chief Nurse Practitioner and the CACC Medical Director, focusing on providing exceptional care to patients with cardiovascular conditions.

Work Environment

This role is an onsite position within a vibrant clinic setting, fostering a collaborative atmosphere that encourages teamwork among Nurse Practitioners, Physicians, Registered Nurses, and various healthcare specialists. The clinic is equipped with state-of-the-art technology to support patient care, including comprehensive electronic medical records (EMR) systems to streamline documentation and communication.

Key Responsibilities

• Conduct thorough patient assessments by obtaining comprehensive histories and performing detailed physical examinations that inform patient care decisions.
• Order, interpret, and analyze diagnostic tests, utilizing evidence-based practices to diagnose and manage both acute and chronic patient conditions.
• Develop and implement patient-centered care plans in collaboration with physicians and other healthcare team members, ensuring that all interventions align with established protocols and standards.
• Engage with patients and their caregivers to provide education on disease processes, treatment options, and self-management strategies to enhance care continuity beyond clinic visits.
• Coordinate care by facilitating hospital admissions and collaborating with inpatient teams to ensure safe and effective discharge planning for patients transitioning from hospital to home.
• Document all patient interactions meticulously within the EMR system to maintain accurate and comprehensive patient records.
• Participate in relevant research protocols, encouraging the integration of the latest clinical findings into practice to elevate the standard of care.
• Maintain a safe and therapeutic environment within the clinic, adhering to all safety regulations and best practices to support patient wellbeing.

Patient Population / Client Focus

The NP will primarily care for a diverse patient population, including adults experiencing complex cardiovascular diseases such as heart failure, pulmonary arterial hypertension, and those undergoing advanced therapies like intravenous medication support and mechanical circulatory assistance. Your insights and interventions will directly contribute to improved patient outcomes in this specialized field.

Qualifications

• A Master’s degree in Nursing with specialized training as a Nurse Practitioner focused on adult care.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses, along with NP board certification.
• Certification in Basic Life Support (BCLS), Advanced Cardiac Life Support (ACLS), and possession of a valid DEA and NPI numbers.
• Proficient computer skills, including experience with electronic medical records and standard office software such as MS Word, PowerPoint, and Excel.

Preferred Experience

While a minimum of two years of experience in a hospital setting is required, preference will be given to candidates with experience in cardiology and who have worked with advanced patient populations. Skills in engaging with patients needing advanced therapies or complex case management will be seen as a strong asset in our collaborative environment.

Compensation and Benefits

We offer a highly competitive salary range of $120,000 to $220,000 annually, based on experience and qualifications.
