Key Responsibilities\n

• Conduct initial and ongoing assessments to evaluate and meet the unique mental health needs of each patient.

• Maintain open lines of communication with supervisors, family members, and other healthcare providers to discuss clients' progress and coordinate care effectively.

• Develop and manage a tentative schedule for appointments and follow-up visits, ensuring adherence to the treatment plan.

• Coordinate any necessary in-home observations or assessments as required, ensuring compliance with our holistic program goals.

• Monitor the implementation of treatment plans and ensure services rendered align with established program standards.

• Participate actively in interdisciplinary team meetings, contributing insights and recommendations to enhance client care.

• Document and maintain comprehensive progress notes monthly and quarterly to track client development and adjust care strategies accordingly.

• Organize and facilitate training sessions for staff to ensure they are equipped with the necessary skills for effective program delivery.

• Perform follow-up interviews and home visits when necessary to ensure continuity of care and support for clients.

• Act as a liaison within the community to promote awareness and access to mental health services.

• Assist in developing training procedures and materials to enhance program effectiveness for clients.

Patient Population / Client Focus\n

You will work with a diverse range of clients, including both adults and youth, each facing unique challenges related to mental health. By using a telehealth approach, you will reach clients who may have difficulty accessing traditional in-person services due to geographical or situational barriers.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Colorado.

• Minimum of two years of counseling experience, with a preference for those with clinical experience in a variety of settings.

• Strong understanding and adherence to legal and ethical standards in mental health practice.

Compensation and Benefits\n

The position offers a competitive salary ranging from USD 90,000 to 100,000 annually, commensurate with experience and caseload availability.
