Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify clients’ mental health needs, strengths, and available resources tailored to end-of-life issues.

• Deliver individualized, family, and group therapy sessions using evidence-based treatment modalities such as Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, and Trauma-Focused Care.

• Develop and execute personalized treatment plans that outline clear goals and interventions based on clients’ values and preferences.
\n
• Collaborate with a multidisciplinary team, including physicians, psychologists, and healthcare professionals, to ensure comprehensive and coordinated care for clients.

• Provide crisis intervention and create strategic safety plans for clients needing immediate support, particularly in emotionally challenging situations.

• Keep meticulous records of client progress and treatment interventions in compliance with ethical standards and legal requirements.

• Facilitate group therapy, psychoeducational workshops, and support groups focused on grief management and coping strategies for clients and their families.

Patient Population / Client Focus\n

In this role, you will primarily serve individuals in a hospice center setting, addressing essential end-of-life issues. Our patient population includes clients of varying ages grappling with terminal illnesses, their families, and caregivers. Your contributions will directly influence their emotional and psychological well-being as they navigate one of life’s most challenging experiences.

Qualifications\n

• Master's degree in Social Work from an accredited college or university.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.

• A minimum of two (2) years of experience in a mental health or outpatient setting, specifically in areas related to grief, trauma, or end-of-life care.

• Strong working knowledge of evidence-based therapeutic techniques and treatment methodologies.
