Position Overview

Peak Performance Physical Therapy is looking for a qualified and dedicated Physical Therapist to join our esteemed team in West Sacramento, California. This is a full-time position that focuses primarily on home care, where you will utilize your expertise to enhance patient recovery and rehabilitation. Your role will include driving positive patient outcomes through personalized therapy plans and comprehensive evaluations.

Work Environment

You will be working primarily in home care settings, providing therapies in a variety of environments that are comfortable and familiar to our patients. The flexibility of our home care model allows us to cater specifically to the individual needs of each patient, establishing a strong rapport that enhances their recovery experience. You will work Monday through Friday, typically from 9 AM to 5 PM, offering a balanced work-life experience.

Key Responsibilities

• Conduct thorough evaluations of patients in their home setting to assess their physical condition, functional capabilities, and rehabilitation needs.
• Design and implement personalized treatment plans based on assessment findings, aligning with each patient's unique health goals.
• Administer a variety of hands-on techniques, including therapeutic exercises that encourage mobility, pain relief, and overall functional recovery.
• Educate patients and their families on techniques for improving body mechanics, enhancing posture, and preventing future injuries.
• Continuously monitor and document patients' progress, adjusting treatment plans as necessary to reflect changing needs and optimize recovery.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive patient care and coordinated treatment strategies.
• Maintain precise and thorough records of patient treatment interventions and outcomes to track progress and inform ongoing care.

Patient Population / Client Focus

Your work will primarily focus on a diverse patient population facing various conditions. Specialties include but are not limited to back and neck pain, joint pain arising from osteoarthritis, sports injuries, post-surgical rehabilitation, and balance challenges. As a Physical Therapist in a home care setting, you can provide tailored interventions that best suit the lifestyle and environment of each patient, ensuring an effective rehabilitation process.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited institution is required.
• Current state licensure as a Physical Therapist is essential.
• A strong commitment to professional development; board certification by the American Physical Therapy Association (APTA) is a plus.
• At least one year of clinical experience in physical therapy; however, candidates with more experience are encouraged to apply.
• Competence in advanced therapy techniques and modalities, with a focus on personalized patient care.
• Strong analytical, problem-solving, and communication skills, enabling effective collaboration with patients and healthcare teams.

Preferred Experience

While a minimum of one year of experience is required, preference will be given to candidates with a robust clinical background that includes treating a variety of conditions related to orthopedic injuries, post-operative care, and chronic pain management. Proficiency in using electronic health records (EHR) systems for documentation is also advantageous.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
