Work Environment\n

This role operates in a remote setting, allowing for flexibility in schedule while engaging directly with families and children across the Elizabeth, New Jersey area. Our Hospice Center provides comprehensive support services that prioritize quality care and compassion. Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting thorough evaluations and assessments to identify the unique needs of children with developmental concerns.
\n
• Participation in the development and implementation of Individualized Family Service Plans (IFSPs) tailored to each child's specific circumstances.
\n
• Monitoring outcomes and tracking progress as a collaborative member of a multidisciplinary team.
\n
• Providing ongoing support and guidance to families, assisting them in enhancing their child’s development through evidence-based practices.
\n
• Utilizing natural environment practices and coaching models to implement intervention strategies effectively.
\n
• Ensuring compliance with all relevant Federal and State regulations.
\n
• Building rapport with children and families to create trusting and positive relationships that facilitate learning.

Patient Population / Client Focus\n

You will primarily work with children from birth to three years old who exhibit developmental delays or disabilities. Our Children’s Services team is dedicated to helping these young individuals achieve milestones related to physical, cognitive, social/emotional, and communication skills. Your expertise will guide family-centered practices, empowering parents and caregivers to support their child’s developmental journey.

Qualifications\n

To be considered for this position, you must meet the following qualifications:

• A Master’s Degree or Doctorate in Social Work.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.

• A valid driver’s license for potential travel during service delivery.
\n

Compensation and Benefits\n
