Key Responsibilities\n

• Conduct thorough and ongoing assessments to determine the unique needs and goals of each patient, formulating personalized treatment plans.

• Maintain open and effective communication with supervisors, fellow team members, family members, and other stakeholders to monitor patient progress and treatment efficacy.

• Provide supervisors with a detailed schedule of appointments, home visits, and observations to facilitate transparent monitoring of patient engagement.
\n
• Engage in meaningful home visits to perform in-depth observations, chart reviews, and data analyses, enhancing the overall treatment approach.
\n
• Ensure that the implementation of individual program plans is both effective and aligns with established protocols.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing insights and collaboration to improve patient outcomes.
\n
• Complete comprehensive monthly and quarterly progress notes, providing clear documentation of patient developments.
\n
• Organize and lead training sessions to familiarize staff and team members with program plans and documentation standards.
\n
• Conduct follow-up interviews and ongoing home visits to continue offering support that aligns with each individual’s evolving needs.

• Maintain organized records, ensuring information is readily accessible to authorized individuals and teams.
\n
• Serve as a liaison with the public and broader mental health community, fostering relationships that support program development goals.

• Assist in creating procedural training materials to continuously enhance program effectiveness for diverse individual needs.

Patient Population / Client Focus\n

In this role, you will have the opportunity to work with a wide range of clientele, including adults and youth facing various challenges related to mental health. Your therapeutic expertise will be valuable in addressing issues such as anxiety, depression, trauma, relationship concerns, and substance misuse. By leveraging telehealth technology, you will help clients navigate their mental health journeys effectively.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Licensed Professional Clinical Counselor (LPCC) in California.
\n
• A minimum of two years of professional counseling experience, ideally within a clinical setting.

• Specialization in family therapy, couples therapy, substance abuse counseling, or trauma-informed care is highly desirable.

• Strong grasp of legal and ethical standards in clinical practice, ensuring compliance with regulations.
\n
• Outstanding interpersonal and communication skills, with the ability to build rapport with a wide variety of clients.

• Demonstrated experience working collaboratively within a multi-disciplinary healthcare team.

• Proficient in using Electronic Medical Record (EMR) software and other relevant digital tools.

Preferred Experience\n

Preference will be given to candidates with specialized training in evidence-based therapies, such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT). Experience working in telehealth environments, as well as cultural competency in addressing diverse populations, is strongly preferred.
