Key Responsibilities\n

• Assist primary care providers in identifying, treating, and managing mental health issues within a multidisciplinary team framework.

• Conduct comprehensive patient intakes focusing on diagnostic criteria and functional evaluations, leading to tailored treatment recommendations.

• Offer brief, focused therapeutic interventions for patients exhibiting mental health concerns, effectively addressing their specific needs.

• Advise primary care providers on whether patients should continue care within the primary setting or be referred to specialty mental health services.

• Initiate follow-up communications with patients to monitor their progress and make timely adjustments to treatment plans as necessary.

• Develop and implement relapse prevention plans for at-risk patients, ensuring they maintain stable function and improve their quality of life.

• Coordinate behavioral health services while linking patients with additional support providers upon patient consent.
\n
• Document patients' treatment progress and maintain accurate clinical records, ensuring compliance with ethical standards and regulations.

• Conduct behavioral health consultations for clinic management to evaluate program effectiveness and suggest improvements.

• Assist patients in adhering to medical treatments recommended by their primary care providers, providing education and coping strategies as needed.

Qualifications\n

Compensation and Benefits\n

Crystal Lake Families & Kids Services values the contributions of its staff and offers a competitive salary range of USD 87,000 to 105,000 per year.
