Work Environment

This position is primarily onsite, where you will deliver therapy directly in patients' homes throughout Cape Coral. The home care setting offers a unique and therapeutic atmosphere, allowing you to develop close relationships with patients and their families while working outside the traditional clinical environment. Key Responsibilities

• Direct personalized physical therapy treatment plans aimed at preventing disabilities, relieving pain, and improving muscle function based on each patient's unique conditions.
• Oversee and mentor Physical Therapy Assistants during their patient interactions to ensure the effective implementation of prescribed therapies.
• Conduct detailed evaluations of clients' physical abilities through advanced diagnostic techniques and make comprehensive assessments to inform treatment plans.
• Assess the home environment thoroughly to provide recommendations aimed at improving the client's independence and safety.
• Provide therapy services as directed by a physician, including the delivery of manual and non-manual therapeutic exercises tailored to the patient's needs.
• Administer physical agents such as traction and massage and evaluate, fit, and adjust prosthetic and orthotic devices as necessary.
• Actively participate in interdisciplinary team meetings, contributing to the collaborative development and review of care plans.
• Monitor and document clients' progress, reporting any changes in condition or response to therapy to ensure timely adjustments to treatment plans.
• Prepare detailed clinical records including initial evaluations, care plans, progress notes, and discharge summaries in compliance with all regulatory standards.
• Educate clients and their families on physical therapy techniques and the proper use and care of assistive devices.
• Complete all required documentation promptly to ensure compliance with agency policies while maintaining the confidentiality of patient information.
• Be readily available to consult with Physical Therapy Assistants in delivering effective patient care.

Patient Population / Client Focus

The primary focus of the Home Care Physical Therapist is to work with a diverse patient population, including those recovering from surgery, managing chronic diseases, or those with physical disabilities. Our patients may include elderly individuals requiring rehabilitation to restore mobility as well as younger adults recovering from injuries. Our tailored approach ensures that each client receives care suited to their specific rehabilitation needs while promoting their independence and comfort.

Qualifications

• A graduate of an accredited physical therapy program, holding a Bachelor's degree in Physical Therapy or its equivalent.
• At least one year of experience as a Physical Therapist in either an acute care or home care setting.
• Current Physical Therapist license in good standing in the state of Florida.
• Current CPR certification.
• Valid driver's license and proof of automobile insurance.
• A creative mindset with an eagerness to lead and innovate in therapeutic practices.

Preferred Experience

• Experience working in a home care environment.

Compensation and Benefits

Full Motion Physical Therapy offers a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience and qualifications.
