Key Responsibilities\n

As a Licensed Professional Counselor, you will:

• Assist primary care providers in identifying and managing mental health and psychosocial issues, enhancing the integration of behavioral health into primary care.

• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, while formulating evidence-based recommendations for treatment goals and plans.

• Provide brief, focused therapeutic interventions for clients needing immediate mental health support, ensuring pharmacological treatments from primary care are appropriately managed.

• Monitor patient progress through regular follow-ups, adjusting treatment plans as necessary to ensure optimal outcomes.

• Collaborate with the primary care team to formulate and implement effective behavior change strategies aimed at patients struggling with both psychological and physical conditions.

• Maintain precise documentation of patient interactions and progress in compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.

• Deliver educational resources and self-management skills to empower patients in their recovery journeys.

• Stay abreast of community resources to effectively support patients and facilitate referrals to specialized mental health care when needed.

• Provide input on behavioral services to clinic management, focusing on program evaluation and outcome monitoring.

Compensation and Benefits\n

The position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience. In addition to the salary, we provide a sign-on bonus as an incentive for new hires.
