Key Responsibilities\n

• Conduct comprehensive patient intakes, including diagnostic assessments and functional evaluations, to formulate appropriate treatment goals in collaboration with primary care providers.

• Provide brief, focused interventions tailored to patients requiring mental health services, thereby enhancing treatment plans and supporting primary care objectives.

• Monitor patient progress, initiate necessary follow-ups, and adjust treatment strategies as needed to ensure optimal outcomes.

• Develop and implement relapse prevention plans for patients at risk, promoting stable functioning within their environments.
\n
• Educate patients on self-management skills and lifestyle choices that enhance their mental and physical health, empowering them to actively participate in their treatment plans.

• Document patient progress diligently and maintain accurate clinical records, ensuring compliance with agency standards and Federally Qualified Health Center (FQHC) guidelines.

• Participate in clinic meetings to discuss behavioral health trends, program evaluations, and consultation on necessary behavioral services.

• Assure collaboration between primary care and behavioral health services, supporting a comprehensive care model.

Qualifications\n

Compensation and Benefits\n

This position offers a competitive annual salary ranging from USD 87,000 to 105,000, commensurate with experience and qualifications.
