Key Responsibilities\n

• Progress Monitoring: Utilize assessment tools and feedback mechanisms to continuously evaluate client progress and adjust interventions as necessary. Document changes in treatment and outcomes accurately and timely.

• Team Collaboration: Work closely within a multidisciplinary team to ensure holistic care for each client. Regularly communicate case details to ensure seamless integration of services and consult on complex cases to optimize outcomes.

Compensation and Benefits\n
