Key Responsibilities\n

Your primary role as an LCPC will involve:

  • Assisting primary care providers in recognizing, treating, and managing mental health and psychosocial issues, enhancing overall healthcare delivery.

  • Conducting thorough patient intakes with a focus on diagnostic and functional evaluations, followed by collaborating with the primary care provider to establish treatment goals and plans.

  • Providing brief, targeted interventions to patients requiring mental health services and referring them appropriately to specialty facilities when necessary.
  • \n
  • Initiating follow-up communications to assess patient progress and adjusting treatment plans based on their evolving needs.
  • \n
  • Developing relapse prevention plans and supporting patients in maintaining stability in their mental health.
  • \n
  • Documenting patient progress and diagnostic information accurately in treatment charts, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.

  • Educating patients on self-management strategies and providing them with the necessary tools for active participation in their recovery process.
  • \n
  • Collaborating with clinic management and other team members to improve behavioral health services and contribute to outcome evaluations.

  • Remaining knowledgeable and compliant with the Code of Ethical Conduct for Clinical Professionals while carrying out additional duties as assigned.

Patient Population / Client Focus\n

You will primarily serve a diverse patient population with varying mental health needs, assisting individuals and families in navigating the challenges posed by mental health disorders. Qualifications\n

To be considered for this role, you must have:

  • A minimum of a Master's Degree in Clinical Counseling.

  • Current Licensed Clinical Professional Counselor (LCPC) license in Illinois.

  • Extensive knowledge of mental health assessment, treatment planning, developmental psychopathology, and psychotherapy techniques.

  • Strong ability to assess individual patient needs and develop effective treatment plans.

  • Proficient documentation skills to maintain up-to-date clinical records and related reports.

  • Capacity to build professional relationships with patients, families, and various stakeholders.

Preferred Experience\n

While not required, bilingual proficiency in English and Spanish is preferred, allowing for broader communication with the diverse population we serve. Experience working within a community health setting and knowledge of available community resources will enhance your effectiveness in this role.

Compensation and Benefits\n

The salary for this full-time position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
