Work Environment\n

This is a remote position, allowing you to work from the comfort of your own home while serving clients in the Sugar Land area. You will collaborate closely with a dedicated interdisciplinary team of healthcare professionals, including nurses, caregivers, and psychiatrists, ensuring a continuity of care for our clients. While the primary work hours are Monday through Friday from 8:00 AM to 4:30 PM, flexibility will be necessary to accommodate client needs, including occasional evening or weekend hours and on-call responsibilities for urgent situations.

Key Responsibilities\n

• Conduct in-depth assessments of seniors' psychosocial needs, evaluating emotional, mental, and social dimensions to guide individualized care plans.

• Collaborate effectively with clients and their families to formulate personalized care plans that identify specific goals, preferences, and resources.

• Provide emotional support and counseling to seniors and their families, facilitating discussions that help navigate challenges such as grief, loss, or major life transitions.

• Advocate for the rights of clients to ensure they have access to appropriate community resources and services, interceding on their behalf as necessary.

• Engage with an interdisciplinary team to foster a comprehensive approach to client care, coordinating efforts to enhance the quality of services delivered.

• Regularly visit clients to monitor their overall well-being, reassessing living conditions, addressing safety concerns, and adjusting care plans as needed.

• Facilitate effective communication and conflict resolution among family members involved in the care process, supporting collaborative decision-making.

• Educate clients and families on available community programs, support groups, and resources to promote social engagement and improve overall well-being.

• Maintain accurate documentation of client assessments, care progress, interactions, and other relevant information using electronic health record (EHR) systems.

• Stay current with social work best practices, policies, and regulations to ensure compliance with ethical standards and continuous professional development.

Patient Population / Client Focus\n

This position focuses on the geriatric population, specifically seniors experiencing a variety of life-limiting illnesses, cognitive decline, or emotional distress. You will work closely with clients in a hospice setting, emphasizing empathy and understanding to enhance their quality of life during sensitive times.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC).

• A minimum of two years of clinical experience, preferably within a community center, hospice, or geriatric setting.

• Extensive knowledge of social work principles, aging-related issues, and community resources available for seniors and their families.

• Strong assessment skills, along with excellent communication and counseling abilities.

• Demonstrated compassion and empathy towards seniors, enabling rapport building with clients and families.

• Proficiency in electronic health record (EHR) systems and various computer applications for record-keeping and communication.

Compensation and Benefits\n

We offer a competitive annual salary ranging from USD 80,000 to 104,000, commensurate with experience and qualifications.
