Key Responsibilities\n

Your day-to-day responsibilities will include:

• Conducting patient intakes and comprehensive diagnostic evaluations to inform treatment plans.

• Providing brief, solution-focused interventions for patients experiencing mental health challenges.
\n
• Collaborating with primary care providers to identify patients who would benefit from specialty mental health referrals.
\n
• Implementing follow-up strategies to assess patient progress and adjust treatment strategies as necessary.
\n
• Developing personalized relapse prevention plans to help patients maintain stability and avoid crisis situations.
\n
• Monitoring the delivery of behavioral health services and coordinating with other healthcare providers, with patient consent, to ensure comprehensive care.
\n
• Documenting patient progress meticulously in treatment charts, ensuring compliance with agency and FQHC guidelines.
\n
• Empowering patients by providing them with self-management skills and educational resources that promote active participation in their recovery.
\n
• Advising clinic management and team members on behavioral health services and potential areas for program evaluation and improvement.
\n
• Supporting patients in adhering to medical treatments initiated by their primary care providers and offering strategies to manage any side effects from medications.

• Upholding the Code of Ethical Conduct for Clinical Professionals in all interactions.
\n
• Performing additional duties as assigned to support the clinic's operational needs.

Patient Population / Client Focus\n

You will primarily work with a diverse patient population in outpatient settings, including individuals facing a variety of mental health concerns. Our patients come from different backgrounds and may present with issues ranging from anxiety and depression to more severe psychological conditions. Your guidance will play an essential role in fostering their emotional, physical, and mental growth.

Qualifications\n

The ideal candidate will possess:

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• A current and valid LPC (Licensed Professional Counselor) license.
\n
• In-depth knowledge of mental health assessment practices, treatment planning, and psychotherapeutic techniques including DSM-IV diagnoses.

• Experience and theoretical understanding of systems in family and group therapy.
\n
• The ability to assess individual patient needs accurately and evaluate the efficacy of services provided.

• Strong communication skills to create and maintain effective relationships with patients, families, and other professionals.

Preferred Experience\n

While not required, proficiency in English and Spanish is preferred to better serve our diverse patient population. Additionally, candidates should exhibit an understanding of community resources available to assist patients effectively.

Compensation and Benefits\n

We are pleased to offer a competitive salary range of USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
