Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues as part of an integrated care team.

• Conduct comprehensive patient intakes focusing on diagnostic assessments, functional evaluations, and treatment recommendations for primary care providers.

• Provide targeted interventions for patients in need of mental health services, adapting strategies to ensure brief yet effective therapeutic sessions.

• Guide decision-making for referrals to specialty mental health services when necessary, being mindful of individual patient needs.
\n
• Implement follow-up processes to monitor patient progress and adjust treatment plans as required, ensuring continuity of care.

• Develop relapse prevention plans and support patients in maintaining stable behavioral and emotional health.
\n
• Identify at-risk patients and collaborate on strategies to avert worsening conditions.

• Document patient interactions accurately in electronic health records to maintain comprehensive treatment charts.

• Maintain active communication with primary care staff to keep them updated on developments in patient care.

• Provide education and self-management strategies to empower patients to take an active role in their treatment journey.

• Offer insights and recommendations to clinic management and team members about program evaluation and behavioral health services.

• Ensure adherence to the Code of Ethical Conduct for Clinical Professionals while completing other duties as assigned.
