Key Responsibilities

As a Licensed Clinical Social Worker at Cognitive Change Center, your primary responsibilities will include:

• Conducting thorough clinical assessments to establish individualized treatment plans based on evidence-based practices, ensuring each patient’s unique psychological, emotional, and spiritual needs are addressed.
• Utilizing therapeutic modalities such as Cognitive-Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) to provide counseling to patients navigating complex emotions associated with terminal illness.
• Maintaining precise clinical documentation in compliance with HIPAA regulations, ensuring confidentiality while managing patient records for analytics and quality improvement.
• Collaborating closely with an interdisciplinary team to formulate and refine holistic care plans that incorporate every aspect of the patient’s well-being.
• Engaging in regular case review meetings to discuss challenges, share insights, and continue growing professionally in a supportive environment.

Qualifications

• A Master's degree in Social Work from an accredited institution, reflecting a strong academic foundation.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey, demonstrating compliance with state regulations.
• At least three years of clinical experience, with a preference for direct practice in hospice or palliative care settings; your experience will significantly enhance the service we provide.
