Key Responsibilities\n

Your role as an LCSW will encompass various critical tasks, including but not limited to:

• Establishing strategic relationships with community services and resources to foster improved aftercare arrangements for patients and families.
\n
• Identifying and navigating client resistance or conflicts through appropriate interventions aimed at reducing tension and prompting constructive evaluations of behaviors and coping strategies.
\n
• Maintaining accurate and comprehensive patient records, documenting all interactions and assessments in compliance with social work standards to facilitate continuity of care.
\n
• Utilizing the latest DSM diagnostic criteria to understand and address disorders in children and adolescents while considering human growth and age-related developmental tasks.
\n
• Conducting thorough psychosocial assessments that incorporate interviews, consultations with healthcare professionals, and chart reviews to inform treatment plans.
\n
• Developing, implementing, and regularly updating personalized treatment plans that reflect the concerns and strengths of patients and their families, including their cultural and spiritual values.
\n
• Facilitating therapeutic groups, delivering education and support to clients, and accurately documenting their engagement and progress.
\n
• Participating in treatment-planning meetings to provide and receive constructive feedback on patient progress and adjustments to treatment strategies.

Patient Population / Client Focus\n

The primary focus of your work will be with patients receiving hospice care, which involves understanding their unique emotional, social, and medical needs. You will also be supporting their families, helping them navigate the complexities of end-of-life care, grief, and bereavement. Your contributions will play a crucial role in ensuring that both patients and families receive the emotional support they need during this significant life transition.

Qualifications\n

To be successful in this role, you should meet the following qualifications:

• Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline.
\n
• Current and valid independent licensure as a Licensed Clinical Social Worker (LCSW) in California.
\n
• Certification in Basic Life Support (BLS) and CPR.
\n
• Demonstrated ability to maintain appropriate professional boundaries and provide empathetic, client-centered care.

• Exceptional organizational skills to maintain accurate documentation and manage multiple responsibilities.
\n
• Strong communication and interpersonal skills, enabling effective collaboration with patients, families, and multidisciplinary teams.

Preferred Experience\n

While not mandatory, the following experiences are preferred:

• Previous work experience in hospice, palliative care, or a similar setting.

• Familiarity with relevant therapeutic techniques such as behavior modification, assertiveness training, and supportive therapy.

Compensation and Benefits\n

The compensation package for this position includes a competitive salary range of $120,000 to $130,000 per year, commensurate with experience and qualifications. In addition, we offer a robust benefits package that includes:
