Key Responsibilities\n

• Collaborate with a multidisciplinary healthcare team to develop and implement patient-centered treatment plans based on comprehensive psychosocial assessments. This includes preparing effective discharge plans following established social work methodologies.

• Conduct thorough patient assessments and execute appropriate interventions, adjusting methodologies based on individual needs and progress. Maintain accurate clinical documentation and progress notes according to clinic policies, ensuring adherence to privacy regulations.

• Provide counseling and crisis intervention services. You will assess emotional and behavioral concerns, offering immediate support and strategizing ongoing care that aligns with patient goals.
