Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient healthcare providers, establishing effective channels of communication.

• Engage with insurance companies regarding pre-certification and utilization management to ensure clients access necessary resources.

• Participate in comprehensive discharge and aftercare planning, identifying suitable follow-up care and support systems for clients.

• Lead therapeutic meal support sessions, guiding clients in developing a positive relationship with food and promoting overall wellness.

• Provide empathetic, safe, and affirming care tailored to each client's unique circumstances and identities, including cultural, socio-economic, and personal backgrounds.

Patient Population / Client Focus\n

Your primary focus will be on population groups served at our hospice center, including older adults facing end-of-life challenges. Your expertise will help facilitate emotional support and mental enrichment while explicitly recognizing the intersecting identities of the clients we serve. You will work collaboratively with families, ensuring that both emotional and mental health needs are comprehensively addressed during this sensitive time.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
