Key Responsibilities\n

• Work collaboratively within the primary care team to recognize, treat, and manage a variety of mental health and psychosocial issues, aimed at achieving holistic patient care.

• Conduct comprehensive patient intakes, including diagnostic assessments and functional evaluations, and communicate treatment goals and recommendations to the primary care provider.

• Administer brief interventions for patients experiencing mental health challenges and design follow-up plans to assess improvements and necessary treatment adjustments.

• Develop tailored relapse prevention strategies, helping patients maintain stable mental health for improved quality of life.
\n
• Selectively refer patients to specialty mental health facilities based on professional assessment of their needs.

• Document patient progress meticulously in treatment charts, ensuring adherence to agency and Federally Qualified Health Center (FQHC) guidelines.

• Empower patients through education and self-management skills to promote their active participation in treatment and recovery.
\n
• Engage in consultation with clinic management regarding behavioral services and evaluation of program outcomes.

• Adhere to ethical standards and professional conduct throughout all interactions and clinical practices.

• Perform additional duties as assigned to support the clinic’s mission and patient needs.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary range between $87,000 and $105,000 annually, commensurate with experience.
