Key Responsibilities\n

• Conduct thorough psychosocial assessments for individuals, adolescents, families, and groups, creating personalized therapeutic plans based on client needs.

• Perform initial comprehensive assessments and triage care efficiently to ensure timely and appropriate interventions.

• Engage in proactive communication with other clinical staff to discuss assessments, treatment strategies, and collaboration on interdisciplinary care plans.

• Demonstrate a robust understanding of diverse patient needs, providing care that encompasses physical, psychological, educational, and safety considerations.

• Manage and facilitate case management for assigned patients by consulting with healthcare providers and community organizations to coordinate holistic care.

• Complete timely and ethical documentation required for all patient interactions and care plans, adhering to departmental standards.

• Implement Cognitive Behavioral Therapy (CBT) and other evidence-based interventions as appropriate, aiding patients in their individualized service journey.

Patient Population / Client Focus\n

The Mental Health Counselor will work with a diverse client base, including children, adolescents, adults, and families facing various psychological challenges. Qualifications\n

• Must hold a valid license as either a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in the State of California.
