Key Responsibilities

• Manage individualized therapeutic care for diverse patient groups, collaborating closely with physicians and other members of the patient care team.
• Conduct thorough evaluations and develop tailored treatment plans that prioritize functional recovery, enhance patient engagement, and ensure an optimal therapeutic experience.
• Deliver hands-on physical therapy treatments and/or supervise physical therapy aides, adhering to compliance standards and best practice guidelines.
• Perform ongoing assessments of functional and rehabilitative status to adapt treatment plans as necessary, ensuring alignment with patient and healthcare team objectives.
• Provide skilled therapy interventions in the patient's home environment when required, ensuring continuity of care.
• Educate patients, caregivers, and families on the proper use of assistive devices and therapeutic exercises to empower self-management of their recovery process.
• Set case management priorities by assessing daily staffing needs and delegating appropriate duties to ensure effective service delivery.

Qualifications

• A valid Physical Therapist license to practice in New York State.
• A Doctor of Physical Therapy (DPT) degree is required.
• Basic Cardiac Life Support (BCLS) certification must be obtained within the first 30 days of employment.
