Key Responsibilities\n

• Conduct comprehensive assessments of clients' needs and develop individualized treatment plans tailored to address specific goals.

• Maintain clear and ongoing communication with multidisciplinary teams, including supervisors, direct care staff, and family members, to discuss patient progress and adjustments to treatment plans.

• Utilize EMR software to document patient interactions, treatment updates, and progress notes, ensuring all records are up-to-date and easily accessible.
\n
• Facilitate weekly virtual sessions, which may include therapy appointments, home visits, and follow-up consultations to monitor clients’ mental health status and adjust plans as necessary.

• Organize and conduct staff training sessions to enhance the knowledge and skills of colleagues regarding program implementation and documentation compliance.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing to collaborative treatment planning and sharing insights based on individual client cases.

• Serve as a liaison with community and public health resources to enhance patient support networks and provide pathways for client referrals.
\n
• Complete regular reviews of treatment efficacy, making data-driven recommendations for improvements when necessary.

The LPC will have the opportunity to work with a diverse caseload, serving both youth and adult populations facing a variety of mental health challenges, including anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Bachelor’s Degree from an accredited institution in Counseling, Psychology, or a related field.

• Active and unrestricted Professional Counselor (LPC) licensure in the state of Kansas.

• A minimum of two years of clinical experience in a counseling setting is required.
