This full-time, onsite position plays a vital role in the Orthopedic Outpatient Unit, where you will deliver critical rehabilitation services to a diverse patient population. Work Environment

The position is based in the home care setting, allowing you to work directly in the community and provide personalized care to our clients in their own environments. As part of our team, you will benefit from a supportive atmosphere that values open communication and teamwork. Our regular business hours are Monday through Friday, from 8 AM to 5 PM, providing a structured schedule that promotes work-life balance.

Key Responsibilities

• Assess patient needs through comprehensive evaluations that include reviewing medical histories, performing specific assessments, and documenting treatment plans tailored to individual goals.
• Implement and monitor treatment protocols effectively, adjusting plans as necessary based on patient progress and feedback, ensuring compliance with evidence-based practices.
• Collaborate with healthcare professionals, providing and seeking input regarding patient care to optimize rehabilitation outcomes.
• Maintain meticulous documentation of patient interactions, from initial evaluations to discharge summaries, ensuring that records reflect the patient's journey and the care provided.
• Facilitate discharge planning and ongoing follow-up care, preparing patients for transitions and continued progress post-treatment.
• Engage actively in team meetings and facility discussions, contributing to the constant improvement of our clinical services.

Patient Population / Client Focus

Our ideal candidate will work with a variety of orthopedic patients, from post-surgical individuals to those managing chronic orthopedic conditions. You will apply your expertise to address their specific needs, offering tailored therapeutic interventions aimed at improving mobility and reducing pain. Your role directly impacts their recovery journey, fostering trust and communication that enhances patient engagement and satisfaction.

Qualifications

• Possess a Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Maintain a current state license as a Physical Therapist in Florida.
• Hold an active American Heart Association Basic Life Support (BLS) certification for healthcare providers.

Preferred Experience

• A minimum of two years of clinical experience, specifically in orthopedic rehabilitation.
• Proven expertise in working with diverse patient populations and the ability to adapt therapeutic approaches accordingly.

Compensation and Benefits
