Key Responsibilities\n

• Collaborate with multidisciplinary healthcare teams to assess client needs and develop individualized treatment and discharge plans that incorporate psychosocial assessments and recognized social work interventions.

• Document clinical assessments, treatment plans, and progress in accordance with departmental policies, ensuring accuracy and confidentiality in all electronic medical records.

Qualifications\n

Compensation and Benefits\n
