Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients' mental health status, needs, strengths, and coping abilities.

• Provide individual, group, and family therapy utilizing evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).

• Develop and implement holistic treatment plans in collaboration with the multidisciplinary team, incorporating patients' goals and preferences.

• Assist patients and families in navigating the complexities of hospice care, ensuring they understand treatment options and available resources.

• Facilitate therapeutic groups and psychoeducational sessions to empower patients and their support systems.

• Advocate for patients' rights and needs, assisting them in accessing community resources and support services as necessary.

• Document clinical interactions and treatment progress in compliance with regulatory standards and organizational policies.

Patient Population / Client Focus\n

You will primarily work with patients facing life-limiting illnesses, ensuring they and their families receive compassionate support through this significant transition. Your expertise will help them cope with emotional challenges, enhance quality of life, and prepare for end-of-life care. You will also provide education and resources to families to help them understand the process and support their loved ones effectively.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LICSW).

Compensation and Benefits\n

Holy Family & Marriage Services offers a competitive salary range of $80,000 to $104,000 annually, commensurate with experience and qualifications.
