Key Responsibilities\n

• Conduct thorough mental health assessments for patients in hospice care, evaluating their psychological well-being and identifying specific concerns and necessary interventions.

• Develop and implement personalized treatment plans in collaboration with patients and their families to effectively manage emotional distress and support end-of-life decisions.

• Deliver therapeutic interventions utilizing evidence-based practices such as cognitive-behavioral therapy (CBT) and grief counseling, specifically tailored to address the unique needs of individuals facing terminal illnesses.

• Facilitate individual and group therapy sessions, fostering emotional expression and coping strategies among patients and their caregivers.

• Provide crisis intervention support in urgent situations, employing de-escalation techniques and coordinating additional resources as required.

• Educate patients and families about mental health issues, treatment options, and available community support services to empower them throughout the hospice journey.

• Maintain detailed documentation of assessments, treatment plans, and interactions in compliance with legal and ethical standards, utilizing electronic health record (EHR) systems for accuracy and efficiency.

• Participate in multidisciplinary team meetings and case conferences, contributing your insights to develop comprehensive care strategies.

You will engage with a diverse patient population that primarily includes individuals facing life-limiting illnesses and their families. Understanding the emotional and psychological challenges these families undergo is crucial. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid LCSW licensure in the state of Illinois.

Compensation and Benefits\n
