Key Responsibilities\n

• Conduct comprehensive assessments of clients’ mental health and psychosocial needs, ensuring to gather relevant history to inform diagnosis.

• Develop personalized treatment plans that reflect each client's unique circumstances, adjusting approaches as necessary based on progress and feedback.

• Implement evidence-based interventions such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy to enhance client well-being.

• Maintain meticulous clinical records and documentation to adhere to confidentiality standards and regulatory compliance, ensuring accurate and up-to-date information on client progress.

• Collaborate with an interdisciplinary treatment team, including psychiatrists, physicians, nurses, and other social workers, to coordinate holistic care strategies for clients.

• Provide psychoeducation to clients and their families, facilitating support groups that strengthen family coping mechanisms and resilience.

• Stay abreast of current research and developments in mental health to incorporate best practices into your work.

Patient Population / Client Focus\n

Your primary focus will be on youth and their families receiving care in a hospice setting. You will work with individuals facing grief, trauma, and anxiety as they navigate the complexities of end-of-life circumstances. Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution is required.

• A valid Licensed Clinical Social Worker (LCSW) license in New York State is mandatory.

• At least two years of clinical experience in mental health, particularly with grief and trauma counseling.

• Proficiency in solution-based psychotherapy interventions, with a strong understanding of trauma-informed care practices.

• Exceptional communication and interpersonal skills to effectively connect with clients and families, as well as collaborate with team members.

• Strong analytical and problem-solving abilities to adapt interventions based on client needs.

Preferred Experience\n

Experience working within a hospice or palliative care environment is highly regarded, as well as familiarity with telehealth practices. Candidates who have developed and facilitated support groups or community outreach initiatives will be at an advantage.
