Work Environment

As a Physical Therapist in our home care division, your work will primarily take place in patients' homes throughout the Freehold area. You will have the opportunity to develop customized treatment plans tailored to each individual, ensuring that care is both effective and personalized. This onsite role typically involves working 40 hours per week with variable weekends as necessary. Key Responsibilities

In your role as a Physical Therapist, you will:

• Conduct thorough examinations and evaluations of patients to assess their functional limitations and disabilities.
• Develop effective, patient-centered treatment plans that incorporate evidence-based practices.
• Provide ongoing consultations to healthcare providers regarding the continuum of care for each patient.
• Maintain a commitment to our customer service standards by interacting respectfully and professionally with all patients, regardless of age or cultural background.
• Implement treatments according to the prescribed plans, re-evaluating progress and making necessary adjustments while educating patients and their families.
• Document treatment sessions meticulously and submit charges for services rendered, ensuring compliance with applicable laws and regulations.
• Collaborate effectively with interdisciplinary team members, including physicians and other healthcare professionals, to optimize patient outcomes.
• Participate in leadership activities such as community outreach, marketing initiatives, and program development, while also engaging in continuous professional education.

Patient Population / Client Focus

The primary focus of this role is to support a diverse range of patients with various conditions, including post-surgical rehabilitation, chronic pain management, and mobility issues related to age or health status. Qualifications

The successful candidate will possess:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• State Licensure in Physical Therapy to practice in New Jersey.
• Basic Life Support (BLS) certification.
• Strong written and verbal communication skills, along with proficiency in word processing and basic computer skills.

Preferred Experience

A minimum of two years of experience in a clinical setting is required. Experience in home care or a community health context is highly desirable, as it equips candidates with the necessary skill set to engage with patients effectively in their home environments.
