Key Responsibilities\n

• Conduct thorough assessments of the biopsychosocial needs of patients, utilizing evidence-based practices to identify appropriate treatment pathways.

• Facilitate case management by coordinating access to resources, assisting clients in completing necessary forms for entitlements and benefits, and ensuring that they receive the essential support to reduce reliance on emergency assistance programs.

• Provide short-term counseling, crisis intervention, and ongoing therapy that is customized to each patient’s situation to promote resilience and recovery.

• Advocate for patients through effective communication with community agencies, ensuring they receive necessary services and support.
\n
• Maintain meticulous patient records and prepare comprehensive reports, adhering to legal and ethical standards while ensuring the confidentiality of sensitive information.

• Assist patients in accessing prescription cost assistance programs, helping them navigate community resources to alleviate financial barriers to their healthcare.

• Coordinate transportation assistance when required, ensuring patients can attend necessary appointments and sessions.

• Participate actively in clinical meetings and case conferences to discuss treatment plans, seek feedback, and support mutual learning.

Patient Population / Client Focus\n

The ideal candidate will work with a diverse clientele, including individuals, couples, and families from various backgrounds and life circumstances. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of one of the following independent licenses is essential: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two years of professional experience in mental health counseling is necessary to ensure that the candidate can contribute effectively in a clinical setting.

Preferred Experience\n

While two years of experience is required, candidates with additional experience in community health settings, telehealth services, or specific modalities such as EMDR (Eye Movement Desensitization and Reprocessing) or cognitive-behavioral therapy (CBT) will be highly regarded. Familiarity with electronic health records (EHR) systems is also a plus.

Compensation and Benefits\n
