Key Responsibilities

• Conduct in-depth evaluations of patients' physical abilities, mobility, and functional limitations to assess their treatment needs.
• Design and implement customized treatment plans that align with each patient's rehabilitation goals, considering their specific challenges and health conditions.
• Monitor and document patient responses to interventions, adjusting care plans as necessary to optimize patient recovery outcomes.
• Ensure compliance with all necessary documentation and reporting required by state and federal regulations while maintaining patient confidentiality.
• Educate patients, their families, and caregivers on therapeutic exercises and techniques, fostering independence and self-management as part of their recovery.
• Collaborate with a multidisciplinary team of healthcare providers to achieve integrated patient care, sharing insights and strategies to improve overall treatment efficacy.

Patient Population / Client Focus

Qualifications

Compensation and Benefits

• The annual salary for this position ranges from $90,000 to $120,000 , commensurate with experience and caseload.
