Work Environment

This position is based onsite in a contemporary outpatient facility located in the vibrant city of San Francisco. Your work schedule will run from Monday to Friday, specifically from 2 PM to 8 PM, allowing for a structured yet personable interaction with patients. Key Responsibilities

• Manage the complete therapeutic care process for an assigned group of patients in coordination with physicians and the healthcare team.
• Conduct thorough patient assessments to develop and implement personalized care plans focused on goal achievement and patient engagement.
• Deliver, or oversee the majority of, physical therapy interventions, ensuring continuity of care across various therapeutic settings.
• Evaluate therapeutic progress and adjust treatment plans based on patient responses and outcomes.
• Educate patients and their families on therapeutic exercises, equipment usage, and home care strategies suitable for ongoing recovery.
• Document all patient interactions, treatment plans, and progress accurately in compliance with legal and ethical standards.
• Collaborate with multidisciplinary teams to streamline patient care processes and enhance overall service delivery.
• Oversee and prioritize daily caseload and coordinate staffing assignments to ensure quality care.

Patient Population / Client Focus

The Physical Therapist will work with a diverse patient population, ranging from those recovering from orthopedic surgeries to pediatric patients with varying conditions. Your role will encompass the assessment of physical function and rehabilitation needs, aiming to improve mobility, reduce pain, and enhance the quality of life for each patient. The emphasis on individualized care will allow you to engage with patients on a personal level, tailoring interventions to meet their unique needs and goals.

Qualifications

• A valid Physical Therapy license in the state of California.
• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of hire.
• No prior experience is required, making this an excellent opportunity for recent graduates to join an established team.

Preferred Experience

• Experience in outpatient physical therapy settings is preferred, but not mandatory.
• Familiarity with electronic health record (EHR) systems and rehabilitation documentation software is a plus.
• Strong interpersonal communication skills, with a focus on patient engagement and education.
