Key Responsibilities

• Conduct thorough evaluations of patients by reviewing their medical history, performing specific assessments, and determining appropriate treatment recommendations based on the findings.
• Communicate evaluation results and treatment plans clearly to both patients and their families, ensuring that they are involved in the goal-setting process.
• Implement treatment protocols tailored to each patient's individual needs, adapting to changes in their health status and the evolving healthcare landscape.
• Collaborate with interdisciplinary healthcare teams to address patients' conditions effectively, documenting any significant changes to adapt care plans as necessary.
• Ensure comprehensive discharge planning and follow-up care, maintaining accurate documentation of patient interactions, treatment progress, and outcomes.
• Demonstrate effective time management skills, adhere to productivity expectations, and maintain punctuality for scheduling and meetings.
• Engage in continuous professional development and education, including serving as a clinical instructor for students.
