Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients to identify their mental, emotional, and social needs.

• Collaborate with medical teams, patients, and their families to develop and implement personalized Plans of Care (POC) that address individual diagnoses and needs.
\n
• Evaluate and adapt treatment plans continuously to reflect changes in patient conditions and family dynamics, ensuring a responsive approach to care.
\n
• Provide individual and group psychotherapy sessions using expressive arts therapy techniques that promote healing and emotional expression.
\n
• Engage in a team-oriented environment to meet departmental goals and objectives, participating in regular case reviews and team meetings.
\n
• Demonstrate clinical competency during field observation sessions with the clinical supervisor, showcasing effective patient/family interaction and care planning.

• Facilitate connections between patients, physicians, and other mental health providers to ensure comprehensive support and interventions as needed.

Patient Population / Client Focus\n

The ideal candidate will work primarily with patients experiencing chronic or life-limiting illnesses. Your role will be crucial in providing emotional support and therapeutic interventions to not only the patients but also their families, helping them navigate the complexities of healthcare and emotional distress associated with chronic illness.

Qualifications\n

• Must possess a valid California license as a Licensed Clinical Social Worker (LCSW).

• A Master’s degree in Social Work is required, along with a minimum of three years of post-licensure experience in a clinical setting.

• Demonstrated knowledge of counseling techniques and crisis intervention, with the ability to carry out independent clinical judgment during direct services.

• Experience in group facilitation, treatment planning, and effective communication with diverse patient populations.

Preferred Experience\n

• Previous experience working with hospice or palliative care patients is highly desirable, as is familiarity with integrative psychotherapy modalities.

Compensation and Benefits\n
